Adult Programs
Overview
Project Kinship’s adult re-entry programs work in unison to support individuals as they transition from incarceration back into the community, providing an integrated network of care rooted in compassion, dignity, and opportunity.
Through a continuum of services that include intensive case management, housing and shelter linkage, substance use and mental health treatment, and in-custody advocacy, we meet people where they are, whether inside correctional facilities, at the jails, or in their own neighborhoods.
Our flagship Adult Re-Entry Success Center (RSC) serves as a central hub for personalized re-entry planning and support, while our South County location (RSC South) ensures access to critical resources and services in South County.
For individuals with high levels of system involvement, our Corrections Cohort provides specialized care coordination and peer-led guidance to reduce recidivism and navigate complex barriers to stability. Those in need of transitional housing and life stabilization are supported through our Behavioral Health Bridge House (BHBH), which offers a safe, structured living environment paired with wraparound behavioral health services.
Together, these programs form a collaborative and holistic re-entry system, working hand-in-hand to ensure each person has access to the resources, relationships, and opportunities they need to heal, rebuild, and thrive.

Anger management
Life skills development
Through rapid housing solutions and a holistic approach, BHBH helps each participant grow, heal, and thrive, laying the foundation for long-term self-sufficiency and wellness.
Corrections Cohort
The Corrections Cohort is a care coordination program designed for individuals with four or more incarcerations within the past 12 months.
Partnering with Orange County agencies and departments, the program aims to reduce recidivism by helping participants navigate the County’s complex system of care.
Through intensive case management and dedicated peer support, our team walks alongside each participant, providing consistent and compassionate guidance through every challenge, and building connections that foster long-term change.
Enhanced Care Management (ECM)
Project Kinship’s Enhanced Care Management (ECM) program for reentry works hand-in-hand with our reentry services to provide intensive, person-centered care coordination for justice-involved individuals transitioning from incarceration back into the community. Rooted in compassion, dignity, and opportunity, ECM ensures that each individual has the health care, resources, and advocacy needed to stabilize and thrive.
Through a continuum of services that includes comprehensive health assessments, appointment coordination, medication education, and linkage to housing, employment, and benefits, we meet people where they are inside correctional facilities, at community hubs, or directly in their neighborhoods. Our team works closely with members to navigate complex health and social service systems, addressing urgent Medi-Cal needs, connecting to substance use recovery programs, and ensuring access to medical and behavioral health providers.
For those with high medical or behavioral health needs, ECM offers specialized care planning and regular follow-up to address chronic conditions, support adherence to treatment, and reduce unnecessary ER visits. By bridging health care with reentry support, we create a coordinated, wraparound system that empowers each individual to rebuild their life, reconnect with their community, and move forward with stability and hope.
